Registration and Record Forms

The Registration and Records Office is responsible for the creation, maintenance and documentation of enrollment information for all enrolled students at our school.  An official student record is maintained by the office to track student information such as, but not limited to,

  • Application/acceptance documentation
  • Class registration and Program Enrollment Information
  • Immunization/Health Record documentation
  • Faculty Evaluations (as appropriate)
  • Requests from External Agencies for Enrollment Information


In addition, this office tracks, verifies and maintains center-wide enrollment data for all programs.

Transcript
– If you were enrolled at REOC and need a transcript, you would need to complete an Information Request Form in order to receive it.  Please print this form from our website and mail/fax to the REOC Registration and Records Office.

Enrollment Verification (CURRENT students only) – If a student needs a letter verifying that they are a current student, you would need to complete an Information Request Form in order to receive an Enrollment Verification Letter.  Please print this form from our website and mail/fax to the REOC Registration and Records Office.

Student Schedule (CURRENT students only) – If you have misplaced your current schedule or need another copy, please utilize the Student Kiosk (located on the first floor of the Andrews Street Building or in the Cafeteria area of the sixth floor of the Sibley Building).  Schedules can be printed from the kiosk at either location anytime either building is open.  If you need direction on how to use the Student Kiosk, please see the Welcome Center/Reception staff in either building or contact the Registration and Records Office for further questions.

GED Diploma/Transcript – If you need an official copy of your New York State GED Diploma or Transcript, please contact the State Education Department, High School Equivalency Program and GED Testing Office at (518) 474-5906.  If you provided the REOC with a copy at time of admission or enrollment, you may request a copy of this transcript by completing an Information Request Form in order to receive it.  Please print this form from our website and mail/fax to the REOC Registration and Records Office.

Change of Address – In order to change your address, please complete a Change of Student Information Form in order to receive it.  Please print this form from our website and mail/fax to the REOC Registration and Records Office.

Change of Name – In order to change your name, you must visit the Registration and Records Office at 161 Chestnut Street, Rochester, NY 14604.  At that time, you must bring written proof of your name change (i.e., marriage certificate, divorce decree, legal documentation from a lawyer, old and new social security cards) as well as complete the Student Change of Information Form which is printable on our website.

Student ID -- Replacement (CURRENT students only) – At time of registration, each student receives an REOC Student ID Card.  If you lose your ID card, a $5.00 fee (payable by cash or money order only) is charged.  If you need a replacement ID, please bring the $5.00 fee along with another form of picture ID to the Registration and Records Office in order to receive a replacement ID.

Certificate – Replacement.  If you have lost your completion certificate, a duplicate certificate can be given to any successful graduate.   You would need to complete an Information Request Form in order to receive it.  Please print this form from our website and mail/fax to the REOC Registration and Records Office.
