What is the next step after I fill out the application?

After completing the application, please submit the following:

• Valid government issued ID
• Current Income Verification*
  • Tax Returns (Federal 1040 or 1040EZ)
  • Unemployment Statement of Benefits
  • Social Security (SSI or SSD)
  • DHS Letter of Referral or DHS Budget Sheet
• Proof of the last 12 month’s New York State Residency
• Social Security Card
• I-94 or Green Card (if you are not a citizen of the United States)
